Birth Records

Birth Record applications will be accepted at the Vital Records Department in the
Carlton County Recorder’s Office at the Carlton County Courthouse.  

Born 1935 & After

A certified birth record for anyone born in Minnesota in 1935 or after may be purchased through the Recorder’s Office.  In order to obtain a certified birth record, certificate eligibility must be shown.  

Born Before 1935

A certified birth record before 1935 may be purchased through the Recorder’s Office. Additional processing may be required. In order to obtain a certified birth record, certificate eligibility must be shown.

Complete the Minnesota Birth Record Application form (PDF); provide the required identification and fee.

By Mail

Complete a Minnesota Birth Record Application (PDF); have the signature verified by a Notary Public. Mail the application form, along with the required fee, to the Carlton County Recorder’s Office

Required Forms

For birth records, download the Minnesota Birth Record application form (PDF), print it, and fill it out. Additionally, the application form may be obtained at the Carlton County Recorder's Office or a form may be sent upon request.  

Required Identification

One of the following documents must be provided in order to meet state requirements:

  • Unexpired picture driver’s license of a U.S. state or territory
  • Unexpired picture identification card of a U.S. state or territory
  • Unexpired U.S. federal, state or local government employee picture identification
  • Unexpired U.S. military picture identification
  • Unexpired U.S. passport

Required Fees

  • $26 First Certified Birth Record
  • $19 Each additional Birth Record of the same record issued at the same time
