Bring your completed Reduced Fee ID form to the Carlton County Motor Vehicle Office to apply for your ID at the reduced ($.50) fee.

• Section A Must be completed and signed by patient in the presence of the vision examiner
• Section B Must be completed and signed by a licensed vision examiner
• Minnesota statutes may require driving restrictions other than those recommended by the licensed vision examiner
• Submit the form to the Carlton County Motor Vehicle Office or mail to the address on the form.
